29 CFR 1926.501(b)(4)(ii):  Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:      Building 1805, Presidio of San Francisco, CA.  On or about March 26, 2013 and at times prior thereto, Centric Construction failed to protect employees from a floor hole exposing workers to serious injuries from the fall hazard.
