OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards. On or about March 26, 2013, at the work site located at Trinidad Drilling Rig #129, 828 West Harwood Rd., Hurst, Texas: employees assigned to work from the derrick man's work platform at approximately 25 to 30 feet were exposed to fire and explosion hazards.

On or about March 26, 2013, at the work site located at Trinidad Drilling Rig #229, 828 West Harwood Rd., Hurst, Texas: an employee was lifted up with the use of Rotzler winch exposing the employee to a fall hazard of approximately 25 to 30 feet.
