On or about March 26, 2013, and at times prior thereto, the general contractor, Plant Construction, did not ensure that the subcontractor, Jerry Thompson Painting, protected its employee from serious physical harm by wearing a hard hat in an area designated by the employer as requiring the use of hard hats.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables running through doorways, windows, or similar openings were not protected from damage. Sharp corners and projections were not avoided. Flexible cords and cables may only pass through doorways or other pinch points, if protection is provided to avoid damage: Presidio Chapel project, Presidio of San Francisco, CA. On or about March 26, 2013, a flexible cable was run through a hole made into a window and metal mesh to provide electrical service to electrical tools and machinery on the opposite side of the wall.

On or about March 26, 2013, and at times prior thereto, the general contractor, Plant Construction, did not ensure that the subcontractor, Jerry Thompson Painting, protected its employee from physical harm by providing effective information and training on hazardous chemicals in their work area including the application and work with StoPrep Architectural Paint Stripper.
