29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 1 Stillings Road, Saugus, MA  On or about 3/28/13, workers were exposed to fall hazards of greater than 20-feet while working from a 5-pitch roof and while working from the second floor rafter platform that was used as a landing to access the roof without a fall protection system.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  Location: 1 Stillings Road, Saugus, MA  On or about 3/28/13, workers were exposed to fall hazards of greater than 8-feet while using unopened portable step ladders to access the following areas of the newly constructed home.  1. Center of the house a Wernher 8-foot A-frame ladder used from the first floor to the second floor. 2. Back of the house a Wernher 6-foot A-frame ladder used from the ground to the first floor.
