29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1.) On or about March 28, 2013, the employer failed to ensure that the rotating chuck and drill bit on the Jet Drill Presses (Model JDP-170X) were adequately guarded to protect the operator. Employees were exposed to hazards from rotating parts and point of operation while operating the machine without a guard.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a)[D] where attached to building surfaces: 1.) On or about March 28, 2013 the employer did not ensure that flexible electrical cords were not attached to building surfaces. A flexible electrical cord was looped around electrical conduit against a wall to keep it in place next to line 2-1 in building #2 of the facility.
