29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a)  Production area - A lockout program was not developed or implemented for equipment such as, but not limited to two Hitachi Seiki Hitec-Turn 20SII lathes, a Cincinnati Milacron Talon 210 lathe, two Haas mills, a Makino mill, eight grinding machines, and a 10 kW laser engraver. On or about 3/28/13.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     a)  Establishment - Employees who operated the Clark propane forklift (model MGCS25MB, serial number 0138MB-0135-6961KOF) were not trained as required by the standard. On or about 3/28/13.   Abatement Note: Forklift operator training consists of a combination of formal instruction (e.g., lecture, discussion, interactive computer based training, video tape, written material, etc.), practical training (demonstration performed by the trainer and practical exercises performed by the trainee), and an evaluation of the operators performance in the workplace.
