29 CFR 1910.67(c)(2)(v):A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   On March 28, 2013 an employee was using an aerial lift to wash the outside of a commercial building without using a body belt and a lanyard.  This condition exposed the employee to a fall up to 15 feet to the ground.
