29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: 1) Location 6280, Jet Electric drill press (SN JD 3099, 606071) used approximately once every two weeks had an unguarded chuck. 2) Desonnter drill press #2 (Model 32602, AFD-60) and Desonnter drill press #3 (Model 32522) used approximately daily had unguarded chucks. 3) MSC milling and drilll press (SN 734473, Model 9512908) used approximately once per week had an unguarded chuck. 4) Location Big G, 16 ' speed drill press (SN 2713000, Company ID 271155) had an unguarded chuck.

29 CFR 1910.1026(h)(1) A hazard was present or was likely to be present from skin or eye contact with chromium (VI); however, the employer did not provide appropriate personal protective clothing and equipment at no cost to employees, and/or did not ensure that employees used such clothing and equipment: For employees who used Alodine 1200S (hexavalent chromium), including but not limited to employees mixing the Alodine 1200S solution or working at the Alodine bath station, and did not wear impermeable gloves and apron, such as butyl or Silver Shield. Employees wore MEDTECH powder free, 15 ml thick, latex examination gloves.
