29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a D-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.    (a) Location - 1275 Turnpike Street, North Andover, Ma located in the Atrium.    On or about 3/27/13, employees were not adequately protected from falling more than 6 feet above a lower level while using snaphooks engaged together.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee.    (a) Location - 1275 Turnpike Street, North Andover, Ma located at the atrium of the building.    On or about 3/27/13, employees were observed using an anchor point that was not rated for 5000 pounds per employee.

(a) Location - 1275 Turnpike Street, North Andover, Ma    On or about 3/27/13, workers were exposed to a free fall greater than six feet below a lower level as a result of improperly using a snaphook as an anchor point.
