29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: (a) On or about March 28, 2013, an area of surficial fungal growth that was suspect mold was observed on the drywall ceiling in the main work area, hallway between the two bathrooms, and the ceiling in the restroom closest to the main entrance. (b) On or about March 28, 2013, Floors in the restrooms of Building J-3 located at 7208 Old Rutledge Pike, Knoxville, TN, were dirty and unsanitary (free from dirt and germs). In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: (a) On or about March 28, 2013, Norfolk Souther Railway did not inspect and document the monthly inspection of the portable fire extinguisher in Building J-3 at the 7208 Old Rutledge Pike, Knoxville TN site. This exposed employees to potential fire hazards. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
