29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Advanced Roofing Services, Inc. at the address of 26611 N. Dixie Hwy. in Perrysburg, OH: The employer failed to ensure that an employee, painting the valley of a steep roof, was protected against falls of approximately 12 feet. Advanced Roofing Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(h)(1)(v), which was contained in OSHA inspection number 313783847, citation number 1, item number 1 and was affirmed as a final order on September 15, 2010, with respect to a workplace located at 550 E. South Boundary Rd. in Perrysburg, Ohio.
