29 CFR 1926.95(c): Personal protective equipment was not of safe design and construction for the work to be performed: a) 569 North Main St., Manchester, CT. Job Site - On or about 3-28-13 - Employee torch cutting a steel chain drive link for the mixer conveyor system suffered 2nd degree burn to his chest when torch turned and burnt through the FR-7A protective jacket. The FR-7A rated jacket is not a primary personal protective jacket for a gas torch cutting operation.

Job Site - On or about 3-28-13 - A 2 ton Jet Electric Hoist was connected to a 1 1/2 ton come-a-long at the main support beam for a system rigged to move the conveyor system under construction.
