29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: On or on dates prior to March 19, 2013 in the maintenance department of Allied Waste employees were exposed to inhalation hazard in that they used respirators on a voluntary basis without being trained on the hazards.

For protection against gases and vapors, the employer did not provide an air-purifying respirator equipped with an end-of-life indicator certified by NIOSH for the contaminant, or a change schedule for canister and cartridges based on objective information or data that would ensure that canister or cartridges were changed before the end of their service life: On, or about, March 19, 2013, on the workroom floor of the facility located at 5645 Moreau River Access Road, in Jefferson City, MO, the employer did not establish a change schedule as to when respirators filters replacement was required.
