29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   On or about March 27, 2013, employees were exposed to falls of approximately 26 feet in height to the concrete level below, while working near and around skylights that were not guarded and employees were not using fall protection, when installing the copper tubing as drain lines for the air condition units.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill.     On or about March 27, 2013, employees were exposed to falls of approximately 26 feet in height to concrete level below, when they were working near and around unguarded skylights and not recognizing the hazard, while installing the drainage system for the air condition units.
