29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE. On or about 19 March 2013, at the job site located at 3225 Robbins Road in Alexander Arkansas, the employer did not ensure employees exposed to fall hazards were protected while engaged in construction activities. As a result the employees were exposed to falls of approximately 11 feet to the unfinished ground below while installing ventilation duct work.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about 19 March 2013 at the job site located at 3225 Robbins Road, Alexander AR, the employer did not ensure employee were trained in fall protection equipment requirements. As a result employees installed vent pipes on the steep roof of a dwelling which exposed the employees to fall of approximately 11 feet.
