29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.     On or about March 22, 2013, and at times prior thereto, the employer did not develop or implement written lockout/tagout procedures for each type of equipment or machine located within the welding shop.  Employees performing maintenance work on the equipment such as but not limited to the Niagara metal press, and the Dayton 1 ton overhead crane were exposed to electrical shock and amputation hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices     On or about March 22, 2013 and at times prior thereto, the employer did not train all authorized and affected employees in the recognition of all energy sources, the purpose and use of a lockout/tag-out system and the general knowledge about the hazards of unexpected release of energy.  Employees performing maintenance work on the equipment such as but not limited to the Niagara metal press, and the Dayton 1 ton overhead crane were exposed to electrical shock and amputation hazards.

29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed.    On or about March 22, 2013, at at times prior thereto, the employer did not ensure that the monthly hook inspections were being documented on all cranes, including but not limited to the Dayton 1 ton crane located in the welding shop. Employees were exposed to struck-by and crushed-by hazards.
