29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Building 640 construction project, Presidio of San Francisco, CA - On or about March 29, 2013, and at times prior thereto, the employer did not protect its employees by training them on the hazards associated with Portland cement. Employees had not been trained on all chemical and materials used at the worksite including the hazards, exposures, PPE, labeling and other forms of warnings on worksite chemicals and materials including Portland cement.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: Building 640 construction project, Presidio of San Francisco, CA - On or about March 29, 2013, and at times prior thereto, thin latex cleaning gloves and gloves with rubber palms and a cotton mesh back side were used instead of gloves made of nitrile or butyl materials for protection against caustic materials such as wet Portland cement.

The mobile scaffold did not have the wheels/casters locked while the work tasks were performed exposing employees to serious fall injuries.
