29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: Building 640 construction project, Presidio of San Francisco, CA - On or about March 29, 2013, and at times prior thereto, the scaffold did not have the wheels/casters locked while the work tasks were performed exposing employees to serious fall injuries.
