29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    At the time of the inspection, the "Bench" press in the Core room was setup to operate with two hand controls that when used to cycle the press did not prevent employees from placing hands into the point of operation.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.213(c)(3):  Hand-fed ripsaw did not have non kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator:    At the time of the inspection, the Heston & Anderson table saw in the pattern shop was used to rip cut lumber without a spreader and a non kickback device to protect the operator from flying material.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    At the time of the inspection, two pedastal grinders in the machine shop were operated without the tongue guard adjusted to within 1/4 inch of the grinding wheel. The OSHA 300A posted for 2012 Task Force Inc. employees was not accurate for the number of cases, number of days, injury and illness types, number of employees, or total hours worked.
