29 CFR 1910.145(f)(4)(vi): Tags were not affixed as close as safely possible to their respective hazards by a positive means such as string, wire, or adhesive that prevents their loss or unintentional removal.  On or about March 28, 2013 the danger tags used were not substantially affixed to prevent unintentional loss or removal.  The red tags in use were made of plain color paper that easily ripped in the wind and environmental conditions thus rendering them in-effective.
