29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: On or about March 27, 2013, and at times prior thereto, in the warehouse areas an employee was using a right angle head grinder without a safety guard. The employee was exposed to laceration and struck by hazards from the unguarded grinding wheel.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about March 27, 2013, and at times prior thereto, in the warehouse areas electrical flexible cords with missing ground pin were used to provide power to other equipment at the facility. Employees were exposed to electrical hazards.
