Rt.21 At Utility Pole # 812A363 and Utility Pole # 812A280: An employee was exposed to falls ranging in height from an estimated 20 to 25 feet to the ground below while working from the bucket of the Versalift mounted to the Ford truck during the installation of the new cable with out any type of fall protection as determined on 3-29-13.
