The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  Plant Wide - The employer did not establish a lockout/tagout program.

29 CFR 1910.212(a)(1):   One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  In the following instances machines were not guarded:  a)  #1 Leg Machine - The rotating conveyor idler drum and shaft were not guarded.  b)  #2 Leg Machine - The rotating conveyor idler drum and shaft were not guarded. c)  #3 Leg Machine - The rotating conveyor idler drum and shaft were not guarded. d)  Mincer Machine - The rotating cylinders and wing nuts were not guarded. e)  Crab Cooker - The nip points between the conveyor belt and sprocket were not guarded. f)   Crab Slaughter Line - The southern waste conveyor rotating shafts and drums were not guarded. g)  Crab Slaughter Line - The northern waste conveyor rotating shafts and drums were not guarded.

29 CFR 1910.333(a):     Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards.  Office Area - The 120 volt electrical circuit, supplying power to the four receptacle outlets, was not deenergized and locked out before the outlet covers were removed.

The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.  Maintenance Area - The portable fire extinguisher was last inspected in 2008.
