29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  a) On or about 04/01/13 an employee was allowed to work on and from a Werner, 28 foot aluminum extension ladder to install leaders and gutters when the ladder had not been inspected by a competent person who could have identified visible defects such as a missing shoe.
