29 CFR 1910.147(c)(5)(ii):  Lockout devices and tagout devices were not singularly identified; devices other than lockout devices and tagout devices were used for controlling hazardous energy; devices were used for other purposes than controlling energy; and did not meet the requirements of items (A) through (D):      On or about March 29, 2013, at the job site.  The lockout/tagout devices did not meet the requirements of the standard for shutting down, isolating, blocking and securing machines and equipment containing hazardous energy, including but not limited to:  Bertsch & Company 9144 shear, punch and roll machine.  Employees were not assured of full protection against such hazards as being caught-by moving equipment or electrical shock.

SPA Pipe & Supply, L.P. dba Smith Pipe of Abilene, was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 314343948, citation number 1, item number 3 and was affirmed as a final order on 02/24/2011, with respect to a workplace located at 7435 US Hwy 277 South, Abilene, Texas 79606.
