29 CFR 1910.243(c)(4): The top half of the wheel of portable grinding machine(s) was not enclosed: Wireline truck: On or about April 1, 2013, and times prior to, employees providing wireline services during the repair of an oilwell used an unguarded angle grinder. This exposed them to the hazard of being struck by grinder wheel fragments.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel: Wireline truck: On or about April 1, 2013, and times prior to, employees providing wireline services during the repair of oilwell used an angle grinder powered through a cordset. No ground fault protection was available.
