An employee was exposed to the gravitational energy of the ram and residual energy of the flywheel when changing the die stamp located within the die.

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, employees were exposed to potentially hazardous stored energy when accessing the hazard area of the Danly 1000 Press #3077 to perform a stamp change. The gravitational energy of the ram and the residual energy of the flywheel was not controlled before employees set up the press.

The original and continuing competence of personnel servicing power presses was not maintained in that employees operated presses that were not guarded properly and/or employees set up die stamps without controlling all hazardous energy sources.

29 CFR 1910.217(c)(3)(vii)(d): Two hand controls used with mechanical power press(es) were not fixed in position so that only a supervisor or safety engineer was capable of relocating the controls: a) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer failed to assure the two hand controls for the Verson Press #3103 (s/n 11119) were located in a fixed position so that only a supervisor or safety engineer could relocate the controls. Employees operating the press were exposed to amputation hazards in that the two hand controls could easily be relocated closer to the point of operation. b) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer failed to assure the two hand controls used to operate the Verson Press #1103 (s/n 10926) were located in a fixed position so that only a supervisor or safety engineer could relocate the controls. Employees operating the press were exposed to amputation hazards in that the two hand controls were able to be easily relocated closer to the point of operation. c) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer failed to assure that the two hand control pedestal used to operate the Minster Press #1078 (s/n 50-8-4236) was in a fixed position. Employees operating the press were exposed to amputation hazards in that the pedestal was not secured and could be moved and operated directly adjacent to the hazard area. d) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer failed to assure that the two hand control pedestal, used to operate the Minster Press #1009 (s/n 50-7 1/2-3801), was in a fixed position. Employees operating the press were exposed to amputation hazards in that the pedestal was not secured and could be moved and operated directly adjacent to the hazard area.

The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operator's knowledge. b) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer did not assure that the operator of the Minster 290 mechanical power press #1078 was instructed in the safe operation of the press in that the selector switch for the press run mode was not supervised. The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operator's knowledge. c) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer did not assure that the operator(s) of the Danly 1000 mechanical power press #3077 was instructed in the safe operation of the press in that the selector switch for the press run mode was not supervised. The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operator's knowledge. d) Defiance Metal Products Co. - Defiance, Ohio: On or about April 2, 2013, the employer did not assure that the operator of the Verson 350 mechanical power press #3103 was instructed in the safe operation of the press in that the selector switch for the press run mode was not supervised.
