Site- An employee operating a Hitachi pneumatic nail gun was not adequately protected from an eye injury where prescription eye wear with side shields was worn and did not have safety lenses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. Site- Employees framing a wall on the second floor were exposed to falls between approximately 10 and 19 feet where fall protection was not utilized.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems,personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  a. Site- When personal fall arrest equipment was utilized to protect employees from falling the employees were not adequately trained in the proper installation of the equipment. Protecta self retracting lifelines were not connected directly to an anchor point as required by the manufacturer.
