29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling. (a) On or about 3/28/13, at the VA clinic room 189 and hallway/passageways, Lasko electric fans approved for residential were used for air circulation due to the strong permeating smell of electrical burning. The VA Southern Nevada Healthcare System was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 311517668, citation number 1, item number 9 and was affirmed as a final order on 7/15/11, with respect to a workplace located at 4801 Linwood Blvd., Kansas City, MO 64128.

(a) On or about 3/28/13, at the VA clinic, group conference room 171, electrical power panel LP-4, circuit breakers were not accurately labeled. The VA Southern Nevada Healthcare System was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(f)(2), which was contained in OSHA inspection number 190598, citation number 1, item number 2 and was affirmed as a final order on 7/18/12, with respect to a workplace located at 916 W. Owens Ave., N. Las Vegas, NV 89030.
