29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)  Production Area - On or about April 3, 2013, where employees engage in servicing and maintenance of metal working machinery, to include changing power transmission belts on the lathes, the employer did not establish a program for the control of hazardous energy prior to employees engaging in servicing and maintenance activities. 29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  (a) Throughout facility - On or about April 3, 2013, where portable fire extinguishers are available for employee use, and an Emergency Action Plan has not been developed requiring the total employee evacuation, employees have not been provided with training regarding basic fire extinguisher use.   Abatement Note: Where extinguishers are provided but are not intended for employee use and the employer has an Emergency Action Plan (EAP) and a Fire Prevention Plan (FPP) that meet the requirements of 29 CFR 1910.38 and 29 CFR 1910.39 respectively, then only the requirements of paragraphs (e) and (f) of this section apply. The EAP must include but is not limited to the elements outlined in 29 CFR 1910.38(c) and the FPP must include but is not limited to the elements outlined in 29 CFR 1910.39(c).

(b) Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a daily examination of each crane for deterioration or leakage in lines, tanks, valves, drain pumps, and other parts of air or hydraulic systems was not conducted for each crane, as required in paragraph (j)(2)(ii) of this section.     (c)  Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a daily visual inspection of hooks for deformation or cracks was not conducted for each crane, as required in paragraph (j)(2)(iii) of this section.     (d)  Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a daily visual inspection of hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations was not conducted for each crane, as required in paragraph (j)(2)(iv) of this section.     (e)  Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a monthly inspection of hooks for deformation or cracks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected was not conducted for each crane, as required in paragraph (j)(2)(iii) of this section.     (f)  Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a monthly inspection of hoist chains including; end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations, with a certification record which includes the date of inspection, the signature of the person who performed the inspection, and an identifier of the chain which was inspected was not conducted for each crane, as required in paragraph (j)(2)(iv) of this section.     (g)  Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a frequent inspection of all functional operating mechanisms for excessive wear components was not conducted for each crane, as required in paragraph (j)(2)(vi) of this section.     (h)  Production Area  On or about April 3, 2013, where employees operate two Overhead Bridge Cranes and nine Jib Cranes, a frequent inspection of rope reeving for non-compliance with manufacturers recommendations was not conducted for each crane as required in paragraph (j)(2)(vii) of this section.
