29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):           On or about March 26, 2013, the employer did not ensure point of operation guarding is in place to protect employees from having any part of their body in the danger zone during operation on the following machines:        a)      On the Green shear, the guard was placed more than one fourth inch above the material exposing employees to the hazards of being caught in the machinery.    b)      On the Green press brake, there was no guard exposing employees to the hazards of being caught in the machinery.   c)      On the Geka ironworker the punch operation was not guarded exposing employees to the hazards of being caught in the machinery.  d)     On the Geka ironworker the shear operation was not guarded exposing employees to the hazards of being caught in the machinery.
