29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Employees were performing roofing work on an 8/12 sloped roof 30 feet above the lower level on the North side of the building and 20 feet above the lower level on the West side of the building without fall protection. This exposed employees to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder: (a) An extension ladder used to access the roof of a new construction residential home, did not extend above the roof eave more than three (3) feet and was not secured at the top. This exposed employees to a fall hazard of 20 feet.
