On or about 03/28/2013, at the job site - an employee was working from a Sky Jack scissor lift model SJIII 3219 equipment number SY060530 with an open side, in that, the chain used as an intermediate guard rail was not connected, exposing the employee to a 16 feet 6 inches fall hazard.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder a. On or about 03/28/2013, at the job site - the extension ladder used to enter and exit the tank was placed at an angle where the foot was at or about 11 feet away from the top support which was about 20 feet high, requiring the ladder foot to be about 5 feet from the base, exposing the employees to a 16 feet 2 inches fall hazard.
