29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) On or about April 2, 2013, in the mechanic shop, the Wilton bench grinder the work rest on the right side was measured at approximately one and one half inch.  Employee(s) exposed to a being struck by the material they are grinding when operating the bench grinder.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to adjust the work rest at one eight inch.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about April 2, 2013, in the mechanic shop, the Wilton bench grinder the tongue guard on the right side was measured at approximately one and five eighth of an inch. Employee(s) exposed to being struck by the material they are grinding when operating the bench grinder.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to adjust the tongue guard one fourth inch.

Employee(s) exposed to an electrical shock.   b) On or about April 2, 2013, the warehouse area, the junction/receptacle box was not secured.  Employee(s) exposed to an electrical shock.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to secure the junction boxes so they do not move.

Employee(s) exposed to an electrical shock.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to repair the outer sheath at the strain relief area.
