Citation was received by employer on 05/03/2013.  Violation occurred on or about 05/01/2013.

29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  125 Dunnell Road, Maplewood, NJ.    Employees working on the pump jack scaffold were exposed to head injuries from roofing materials and tools from roofers above as the employer had not provided for hardhats to be worn.                                                Violation observed on or about 04/01/2013.

29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above a lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a)  125 Dunnell Road, Maplewood, NJ.    Workers on the Alum-A-Pole pump jack scaffold platform were exposed to falls of 24 feet to the ground below as the ends of the scaffold platform were not properly guarded and workers were not using personal fall arrest systems.                                                            Violation observed on or about 04/01/2013.

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a)  125 Dunnell Road, Maplewood, NJ and at the Corporate Offices at 70 Scott Avenue, South Amboy, NJ.    Laborer doing siding work using a pump jack scaffold system had not received instruction on proper scaffold use and hazard recognition by his employer exposing him to injury from falls or scaffold collapse.                                                  Violation observed on or about 04/01/2013.
