29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On or about March 29, 2013, an extension cord plugged into the outlet in the basement was missing the grounding pin. The extension cord was used to energize a chop saw that was double insulated.
