The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a.  At the work site, employees operated fork lift trucks to move large cardboard bins of electronic parts on pallets from the receiving, sorting and shipping areas.  The employer did not have written documentation of employee training on the operation of fork lift trucks including a method of observation and documentation of drivers' competency.

Employers shall have a material safety data sheet in the workplace for each hazardous chemical which they use.  a.   At the work site located at  2899 Morse Road, Columbus, Ohio, employees received at a counter, at a delivery dock, and from multiple contracted collection sites, various electronics for sorting and processing for shipment to recycling centers.  The employer did not have material safety data sheets available for employees who handled the hazardous chemicals found in the recycled items.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  a.  At the work site, employees received electronic devices from a counter and contracted drop-off points for sorting and palletizing to send to recyclers.  On the day of inspection, the employer did not have any employee training materials or employee training records about the hazards of materials being handled.
