Site- Workers were exposed to multiple fall, struck by, and electrical hazards where the controlling contractor failed to ensure that a competent person conducted frequent and regular inspections of the jobsite, materials, and equipment.

29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof:    a. Site- Workers were not protected from fire hazards where gasoline and flammable adhesives were present at the site and the controlling contractor failed to provide fire extinguishers. 29 CFR 1926.405(a)(2)(ii)(B): Branch-circuit conductors will not be laid on the floor:    a. Site- Workers were not protected from electric shock and burns where a 220 volt branch circuit that originated in panel board mounted to a pole at the street traveled across the site on the ground and into the building under construction to another panel board. The branch circuit was not protected from damage while exposed on the ground and an all terrain forklift was found running over the branch circuit.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. Site- Workers were not protected from falls of up to approximately 30 feet from unguarded roof edges, floor edges, and stair openings where the controlling contractor failed to ensure fall protection was utilized.
