29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:     a.  On or about April 4, 2013 - at the above addressed jobsite, employees working inside an excavation which consisted of several depths ranging between six (6) feet six (6) inches to seven (7) feet six (6) inches were not provided with an adequate protective system, thereby exposing the employees to cave in hazards.     HAYES INDUSTRIES, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315155903, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 17, 2011 WITH RESPECT TO A WORKPLACE LOCATED AT COMMERCE CENTER AND DRALLE ROAD, UNIVERSITY PARK, IL  60466.
