29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On April 4, 2013, employees were not protected from a fall while working on a residential property, exposing employees to a fall hazards between 12 and 23 feet above grade. Instance 1. One employee was not protected from a fall as he walked on the plywood sheeted roof. He was exposed to a fall of 23 feet. Instance 2. One employee was not protected from a fall as he framed a small dormer from the roof. He was exposed to a fall of 12.7 feet. Instance 3. One employee was not protected from a fall as he pulled rafters through an unguarded window, exposing him to a fall of approximately 15 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about April 4, 2013, employees engaged in residential construction were not provided training to recognize and minimize the hazards associated with with a fall, exposing employees to the hazards associated with a fall. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
