29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    a) 401 Rocky Run Parkway, Wilmington, DE: Employees used a 100 foot extension cord without a ground pin.  The cord was used to service a Pro Team Coach vac, 120 volts, Model: CM-300, Serial: 03-070026, portable vacuum cleaner.  The employees used the cord daily to service various equipment used at the site.  This exposed the employees to electrical hazards.  The violation was observed on or about April 4, 2013.

The cord was used to service a Pro Team Coach vac, 120 volt, Model: CM-300, Serial: 03-070026, portable vacuum cleaner.  The employees use the cord on a daily basis to service various equipment throughout the site.  The violation was observed on or about April 4, 2013.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:       a) 401 Rocky Run Parkway, Wilmington, DE (Stock room): Employees used an 120 volt electrical portable flexible cord with a damaged strain relief.  The MAC Battery Charger Model: V240 cord is used to charge the Speed Scrub 2401 floor cleaner.  This exposed the employees to electrical hazards.  The violation was observed on or about April 4, 2013.             Diversified Maintenance Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(g)(2)(iii), which was contained in OSHA inspection number 311813448, citation number 1, item number 2 and was affirmed as a final order on 02/13/2012, with respect to a workplace located at 401 Rocky Run Parkway, Wilmington, DE.
