29 CFR 1910.27(d)(3): The side rails of through or side-step ladder extensions did not extend 3 1/2 feet above paraphets and landings.  (a) On or about March 27, 2013, the ATSS-SSC employees are required to go onto the roof of the air traffic control tower cab.  There are two fixed ladders from the cab to  the roof.  Both ladders did not extend 3 1/2 feet above their landings.

29 CFR 1904.32(a)(2): 29 CFR 1904.32(a)(2): At the end of each calendar year, an annual summary was not created for injuries and illnesses recorded on the OSHA 300 Log:  (a) On or about March 27, 2012, the employer at the ATCT had not created an OSHA 300A summary for OSHA Log years 2010 through 2012.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a) On or about March 27, 2013, procedures for the chiller (CH-1B), Boiler (B-2B) and ASR (Equipment 44 E/G) were provided for review.  It was found that machine specific shutdown procedures were not present, the specific procedures to release stored energy were not present, and the specific procedures to verify the control of hazardous energy were not present.  These include how to test for energy and the specific procedures for a restart.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) On or about March 27, 2013, two containers with 5 gallons each of Nalco 8338 were found in the boiler room.  The container warning identified that contents were corrosive.  No eyewash was available in the boiler room where the chemical is added to the boilers.
