It was found that machine specific shutdown procedures were not present, the specific procedures to release stored energy were not present, and the specific procedures to verify the control of hazardous energy were not present.  These include how to test for energy and the specific procedures for a restart.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) On or about March 27, 2013, two containers with 5 gallons each of Nalco 8338 were found in the boiler room.  The container warning identified that contents were corrosive.  No eyewash was available in the boiler room where the chemical is added to the boilers.
