On or about April 4, 2013 - at the above addressed jobsite, an employee using pneumatic nail gun to perform carpentry operations did not wear safety glasses, thereby exposing the employee to the eye injuries.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about April 04, 2013 - at the above addressed jobsite, employees performing carpentry framing operations from the attic deck, approximately eleven (11) feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.
