29 CFR 1926.153(h)(5): Valves on LP-Gas containers having water capacity greater than 50 pounds (nominal 20 pounds LP-Gas capacity) were not protected from damage while in use or storage. a) Fed-Ex, 40 Kennedy Road, South Windsor, CT. Job Site - On or about 4-4-13 - Employee operating a Hyster industrial fork lift was exposed to a fire/burn hazard from 40lb propane tank that was not prevented from rotating on the lift by a positioning pin that was missing.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. a) Fed-Ex - 40 Kennedy Road, South Windsor, CT. Job Site - On or about 4-4-13 - Employee was exposed to a fire/burn hazard while operating an industrial fork truck with the propane tank retaining pin missing. The missing pin would allow the tank to rotate/spin and break the propane line and valve causing a fire/burn hazard. As per ANSI B56.1-1969 Section 606A defective fork trucks shall be immediately taken out of service until repaired.
