29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:     On or about April 3, 2013, at the above addressed site, an employee performing masonry work filling voids in concrete blocks with concrete mix was being supported by the bucket of a front end loader at the work level.

29 CFR 1910.1200(g)(8): The employer shall maintain in the workplace copies of the required material safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s):  On or about April 3, 2013, at the above addressed site, the employer did not maintain copies of the required Material Safety Data Sheets (MSDS) for each hazardous chemical in the workplace including but not limited to Portland Cement and Crystalline Silica contained in a concrete mix and Diesel used to fuel a front end loader.
