Employees were exposed to a struck-by hazard while operating a Yale HF-018 Model GLC155CANGBV100 fork truck, rated at 16,900 pounds maximum lift capcity. employees with supervisors present lifted and maneuvered a parts furnace model 62-ADH-650 S that weighed 39,000 pounds.

Employees in the small die room were exposed to a struck by hazard from possible failure of load hooks because monthly inspections for deformities and cracks were not properly documented. The hook identifier is not noted in inspections only the hoist to which they are attached are noted.
