29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about March 26, 2013, employees were exposed to a fall hazard of greater than 20 feet when they were observed to be standing on the mid-rail of a vertical (scissor) lift rail guard.  The employer had failed to provide the employees with training on fall hazards and the proper use of the scissor lift fall protection, i.e., standing firmly on the floor of the basket.
