A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a) East side of facility, between building and Route 73 - Employees working in the approximate 7-11 foot deep trench did not have a ladder or other proper means of egress, on or about 04/24/13.

29 CFR 1926.651(j)(1): Adequate was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face. Such protection shall consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection: a) Between building and the roadway at 311 Route 73 - Employees were working in an approximate 12-14 foot deep trench x 50 feet long and 5-6 feet wide with spoil coming up to the edge of the excavation, on or about 04/24/13.
