29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    (a)  Compressed gas cylinders, which were full of oxygen, were not properly supported to prevent them from being knocked over as required by Compressed Gas Association Pamphlet P-1-1965, Section 3.4.4.     (b)  Compressed gas cylinders, which were empty of oxygen, were not properly supported to prevent them from being knocked over as required by Compressed Gas Association Pamphlet P-1-1965, Section 3.4.4.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a)  The employer had not performed periodic inspections of employees who performed lock out/tag out of hazardous energy as part of their job duties in over 3 years.    Mancor Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(ii), which was contained in OSHA inspection number OIS 86557 (IMIS 312738065), citation number 2, item number 1, and was affirmed as a final order on January 25, 2010, with respect to workplaces located at 1008 Leonhard Street and 600 Kiser Street, Dayton, OH.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    (a)  A spray bottle on a table in the laser area was not labeled with the contents of the container or the hazards of the chemicals it contained.       Mancor Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(f)(5)(i), which was contained in OSHA inspection number OIS 85992 (IMIS 12738099), citation number 2, item number 2, and was affirmed as a final order on January 25, 2010, with respect to workplaces located at 1008 Leonhard Street and 600 Kiser Street, Dayton, OH.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
