29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used:    a. On or about April 2, 2013, at the above addressed jobsite, employees were exposed to falls when accessing/egressing the ladder jack scaffold platforms which were set on the outside of the support ladders. Access/egress required the employees to climb/crawl over the scaffold platform to get on or off the support ladders rather than stepping from or onto a seperate ladder.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system:    a. On or about April 2, 2013, at the above addressed jobsite, employees working from the ladder jack scaffolds were exposed to falls of approximatley 12 feet due to the lack of a personal fall arrest system (body harness with lanyard attached to a lifeline).

On or about April 2, 2013, at the above addressed jobsite, employees had not been trained to recognize the hazards involved and the safeguards required to protect themselves while working from ladder jack scaffolds.
